OSHA 30 HOUR CARDS

Effective with the OSHA 30 hour class that ended December 10, 2009 you will be required to pick up your OSHA 30 Hour card at the Training Center in person or they will be distributed at the next meeting of the local. You will be required to sign a form stating you have received your card. There currently is no expiration date on this certification.

OSHA 30 Hour replacement cards

I lost my 10 hour or 30 hour student completion card. How do I get a replacement?

Records of your attendance at training are required by law to be kept at the Training Center and by the Trainer for a period of 5 years. In order to replace a lost card the Training Center has to request a replacement card from OSHA. The Training Center must provide  your name, training date, and type of card to OSHA verifying your attendance. Please make every attempt to keep track of your card. 

A $100 fee will be charged to issue a replacement card.

 OSHA cards can only be replaced within 5 years of the issuance of your original!

If a period of 5 years has passed since the original issued date the card cannot be replaced. Unfortunately you will be required to sit for the class again.
